Job Title: LPN/LVN – Long-Term Care (LTC)

Location: Wayland, NY
Duration: 13 Weeks
Schedule: Night Shift (4x10 Hours)
Shift Hours: 10:00 PM – 8:00 AM

Compensation

  • Traveler Pay: $46–$48/hr (based on experience)
  • Local Pay: $39–$40/hr (based on experience)

Position Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/LVN) to provide quality nursing care in a long-term care setting. The LPN will work under the supervision of a Registered Nurse (RN) and interdisciplinary healthcare team to support individuals requiring ongoing medical care and assistance with daily health needs.

The ideal candidate will have strong clinical skills, excellent documentation abilities, and a commitment to providing safe, patient-centered care while maintaining compliance with all healthcare regulations and standards.

Responsibilities

  • Administer medications, including controlled substances, in accordance with physician orders and facility policies.
  • Monitor and document medication effectiveness, patient responses, and compliance with treatment plans.
  • Obtain and record vital signs, conduct physical assessments, and monitor patient health status.
  • Perform wound care, dressing changes, and other basic nursing procedures as assigned.
  • Conduct routine health screenings and assist with preventative care initiatives.
  • Respond appropriately to medical emergencies and changes in patient condition.
  • Accurately document patient assessments, treatments, and progress in the medical record.
  • Collaborate with Registered Nurses, physicians, and other healthcare professionals to support individualized care plans.
  • Maintain compliance with facility policies, safety procedures, and regulatory requirements.
  • Provide compassionate care while promoting patient dignity, comfort, and well-being.
  • Assist with patient education and support services as appropriate.
  • Participate in interdisciplinary team meetings and ongoing quality improvement initiatives.

Required Qualifications

Education

  • Associate Degree in Health Science or related field preferred.

Licensure & Certifications

  • Active New York State LPN License required.
  • Current BLS Certification required.

Experience

  • Previous Long-Term Care (LTC), Skilled Nursing Facility (SNF), developmental disability services, or related nursing experience preferred.
  • Experience administering medications and performing routine nursing procedures.

  • Knowledge of medication administration and documentation practices.
  • Excellent communication and interpersonal skills.
  • Ability to respond effectively in emergency situations.
  • Strong organizational skills and attention to detail.
