Job Title: RN - Case Manager (Travel)
Location: Hyannis, MA
Type: Contract
Shift & Schedule: Monday–Friday | 40 Hours/Week | 8-Hour Shifts | Rotating Weekend & Holiday On-Call

Position Overview:
Navitas Healthcare, LLC is seeking RN - Case Manager for an exciting Travel opportunity in Hyannis, MA.

Responsibilities

  • Lead and oversee daily Case Management department operations
  • Supervise clinical resource management and discharge planning processes
  • Ensure compliance with CMS regulations and organizational standards
  • Develop and implement process improvement initiatives
  • Collaborate with interdisciplinary teams to improve patient outcomes and care coordination
  • Monitor resource utilization, length of stay, and quality metrics
  • Assist in the development and presentation of continuing education programs
  • Provide leadership, mentorship, and support to case management staff
  • Participate in rotating weekend and holiday on-call coverage
  • Maintain accurate documentation and reporting within Epic EMR

Requirements

  • Bachelor of Science in Nursing (BSN) required
  • Current Massachusetts RN License required
  • Minimum 5 years of Acute Care Case Management experience required
  • Minimum 2 years of Process Improvement experience required
  • Strong knowledge of CMS regulations
  • Epic EMR experience required
  • Excellent analytical, organizational, interpersonal, and writing skills
  • Strong computer proficiency and communication skills

Preferred Qualifications

  • Master’s Degree preferred
  • Management or supervisory experience preferred
  • Certified Case Manager (CCM) certification preferred
  • Clinical Documentation Integrity certification preferred
