Job Title: Licensed Master Social Worker (LMSW)

Location: New York, NY
Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Weekly Hours: 40 Hours
Duration: 13 Weeks

We are seeking an experienced Licensed Master Social Worker (LMSW) for an inpatient hospital assignment in New York, New York. This opportunity is ideal for a compassionate and skilled social worker with experience in psychosocial assessments, supportive counseling, discharge planning, and care coordination within an acute care environment.

The LMSW will work closely with interdisciplinary healthcare teams to support patients and families throughout hospitalization and discharge planning processes.

Responsibilities

  • Conduct psychosocial assessments for patients and families

  • Provide supportive counseling and therapeutic interventions

  • Coordinate discharge planning and continuity of care services

  • Facilitate referrals to community resources and support services

  • Collaborate with interdisciplinary healthcare teams to support patient care plans

  • Assist patients and families with crisis intervention and care coordination

  • Maintain accurate clinical documentation and case records

  • Support patients through medical, emotional, and social challenges during hospitalization

  • Ensure compliance with hospital policies and patient confidentiality standards

  • Participate in interdisciplinary meetings and patient care discussions

Requirements

  • Master’s Degree in Social Work (MSW) required

  • Active New York LMSW License required

  • Primary Source Verification required

  • Minimum 1 year of LMSW experience required

  • Hospital-based social work experience strongly preferred

  • Experience in inpatient medicine, med/surg, or rehabilitation settings preferred

  • Strong discharge planning and referral coordination experience required

  • Excellent communication, counseling, and documentation skills

  • Ability to work effectively in a fast-paced healthcare environment
