Job Title: Licensed Practical Nurse (LPN) – Long Term Care

Location: Falmouth, MA
Duration: 13 Weeks
Schedule: 8-Hour Shifts | Mixed Day & Evening Rotations

Shift Timings:

  • 7:00 AM – 3:30 PM

  • 3:00 PM – 11:30 PM

  • Every Other Weekend Required

  • Holiday Rotation Required

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a Long-Term Care team in Falmouth, Massachusetts. The LPN will provide direct nursing care to residents while supervising day-to-day nursing activities performed by nursing assistants in accordance with federal, state, and local regulations.

The ideal candidate will be committed to maintaining the highest standards of resident care, safety, dignity, and service excellence in a skilled nursing environment.

Responsibilities

  • Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies

  • Administer medications and treatments accurately while verifying resident identity prior to administration

  • Perform skilled nursing procedures including catheterization, tube feedings, suctioning, wound care, colostomy care, specimen collection, and range of motion exercises

  • Monitor and record vital signs including temperature, pulse, respirations, and blood pressure

  • Observe, document, and report changes in resident condition

  • Complete and maintain accurate documentation related to admissions, transfers, discharges, treatments, and resident progress

  • Transcribe physician orders to charts, medication records, and care plans

  • Receive and accurately document physician telephone orders

  • Supervise and direct nursing assistants and support staff during assigned shifts

  • Assist with staffing coordination and assignment delegation

  • Collaborate with physicians, Nurse Supervisors, dietary staff, and interdisciplinary teams regarding resident care

  • Provide education and healthcare instruction to residents and family members

  • Ensure resident rights, dignity, and safety are maintained at all times

  • Participate in facility inspections and regulatory surveys as needed

  • Maintain strict confidentiality of resident information and protected health information (PHI)

Requirements

  • Graduate of an approved Licensed Practical Nursing (LPN) program or accredited nursing program

  • Current, active, and unrestricted Massachusetts LPN License required

  • Current BLS/CPR and AED Certification required

  • Knowledge of nursing practices, long-term care regulations, and resident care standards

  • Strong clinical, organizational, leadership, and documentation skills

  • Ability to work independently and make sound clinical decisions

  • Ability to supervise staff and collaborate within a multidisciplinary team

  • Ability to lift a minimum of 50 pounds and meet physical requirements of the role

  • Long-term care or skilled nursing facility experience preferred

  • Excellent interpersonal and communication skills
