Location: South Dennis, MA
Contract Duration: 13 Weeks
"Navitas Healthcare, LLC" is seeking a Registered Nurse (RN) for a Home Health assignment in South Dennis, MA. The RN will provide skilled nursing care, complete patient assessments, coordinate care plans, and support patients in a home health environment while ensuring quality outcomes and compliance with clinical standards.
Perform comprehensive nursing assessments upon patient admission
Develop, implement, and revise individualized patient care plans
Coordinate care according to case management objectives
Evaluate patient conditions and document skilled nursing care
Participate in interdisciplinary patient case conferences
Supervise Home Health Aide care plans every 14 days
Notify physicians and care providers of significant patient changes
Complete admissions, discharge paperwork, and clinical documentation timely
Obtain and clarify physician orders for all care provided
Educate patients and caregivers regarding treatment plans and care needs
Coordinate patient discharges and community resource referrals as appropriate
Maintain clear, concise, and accurate documentation
Active Massachusetts RN License required
Minimum 1 year of nursing experience required
OR 6 months of nursing experience with BSN
CPR Certification required
Strong assessment, documentation, and communication skills
Ability to work independently in a home health setting
Home Health experience preferred
IV therapy experience preferred
BSN preferred
Computer skills preferred
