Job Title: RN – Home Health

Location: South Dennis, MA
Contract Duration: 13 Weeks


Job Summary

"Navitas Healthcare, LLC" is seeking a Registered Nurse (RN) for a Home Health assignment in South Dennis, MA. The RN will provide skilled nursing care, complete patient assessments, coordinate care plans, and support patients in a home health environment while ensuring quality outcomes and compliance with clinical standards.


Key Responsibilities

  • Perform comprehensive nursing assessments upon patient admission

  • Develop, implement, and revise individualized patient care plans

  • Coordinate care according to case management objectives

  • Evaluate patient conditions and document skilled nursing care

  • Participate in interdisciplinary patient case conferences

  • Supervise Home Health Aide care plans every 14 days

  • Notify physicians and care providers of significant patient changes

  • Complete admissions, discharge paperwork, and clinical documentation timely

  • Obtain and clarify physician orders for all care provided

  • Educate patients and caregivers regarding treatment plans and care needs

  • Coordinate patient discharges and community resource referrals as appropriate

  • Maintain clear, concise, and accurate documentation


Required Qualifications

  • Active Massachusetts RN License required

  • Minimum 1 year of nursing experience required

    • OR 6 months of nursing experience with BSN

  • CPR Certification required

  • Strong assessment, documentation, and communication skills

  • Ability to work independently in a home health setting


Preferred Qualifications

  • Home Health experience preferred

  • IV therapy experience preferred

  • BSN preferred

  • Computer skills preferred
