Job Title: Hospice RN Case Manager
Location: Grand Junction, CO
Type: Contract | 12 Weeks (With a strong possibility of extension)
Shift & Schedule: Variable Days | 40 Hours Per Week
Pay Rate: $1,800/week to $2,200/week
Position Overview:
Navitas Healthcare, LLC is seeking a Hospice RN Case Manager for an exciting Travel or Local job in Grand Junction, CO.
Responsibilities
Manage and coordinate care for an assigned hospice patient caseload.
Conduct comprehensive patient assessments and develop individualized care plans.
Provide education and support to patients, families, and caregivers.
Collaborate with physicians and interdisciplinary healthcare teams.
Deliver expert symptom management and end-of-life care.
Monitor patient progress and adjust care plans as needed.
Maintain accurate and timely clinical documentation.
Provide care in patients' homes, assisted living facilities, nursing homes, and other community settings.
Participate in occasional on-call and weekend coverage as assigned.
Required Qualifications
Graduate of an accredited Registered Nursing Program.
Active Colorado RN License or Compact RN License.
Current BLS Certification.
Minimum 2 years of RN experience.
Strong assessment, critical thinking, and communication skills.
Preferred Qualifications
Previous Hospice, Home Health, Palliative Care, or Case Management experience.
