Job Title: Occupational Therapist
Location: St. Johnsbury, VT
Duration: 13 Weeks
Shift Details:
• Day Shift – Monday through Friday
• Up to 40 Hours per Week
Position Overview:
Navitas Healthcare, LLC is seeking Occupational Therapist for an exciting Travel or Local job in St. Johnsbury, VT.
Responsibilities:
• Provide occupational therapy evaluations and treatment plans in patient homes
• Assist patients in improving functional independence and safety
• Develop individualized care plans based on patient needs and goals
• Educate patients and caregivers regarding therapy programs and adaptive strategies
• Collaborate with nurses, physicians, and interdisciplinary care teams
• Maintain accurate and timely documentation of patient care
• Support home care and hospice patients with compassionate, patient-centered care
Requirements:
• Active Vermont Occupational Therapist License required
• Current BLS Certification required
• Valid Driver’s License and reliable vehicle required
• Home health or hospice experience preferred
• Strong communication, organizational, and independent working skills required
