Assignment Overview

We are seeking an experienced Home Health Physical Therapist for a 13-week travel assignment serving Seaside, Oregon and surrounding communities. This role is ideal for a therapist who is comfortable working independently in a home health setting while delivering high-quality patient-centered care.

Assignment Details

• Shift: Day Shift – 5x8 Hours
• Schedule: 8:00 AM – 4:30 PM
• Contract Length: 13 Weeks
• Location: Seaside, Oregon and surrounding areas

Patient & Productivity Information

• Home Health setting with 1:1 patient care
• Average of up to 6 patient visits per day
• Productivity expectation: 30 points standard
• System designed to minimize travel time between patient visits

Job Responsibilities

• Evaluate and treat patients in their homes according to physician care plans
• Develop individualized treatment plans focused on mobility, strength, balance, and functional independence
• Complete OASIS documentation accurately and timely
• Educate patients and caregivers on therapy goals, exercises, and safety techniques
• Coordinate care with interdisciplinary healthcare teams
• Maintain compliance with home health regulations and documentation standards

Requirements

• Minimum 1 year of Skilled Home Health experience required
• First-time travelers accepted
• BCLS certification required
• OASIS experience required
• Pending license accepted
• Must have reliable personal transportation
