Job Title: Licensed Practical Nurse (LPN) – Long Term Care (LTC)

Location: New Lisbon, WI 53950

Contract Duration: 13 Weeks (Long-Term Contract)

Shift Details

• Full-time PM Shift: 2:00 PM – 10:00 PM
• Guaranteed Hours: 36 hours per week
• Weekend Requirement: Every other weekend

Job Overview

We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care (LTC) setting. The ideal candidate will be experienced in supporting elderly residents, administering medications, and collaborating with healthcare teams to ensure patient safety and comfort.

Key Responsibilities

• Administer medications safely following the “5 Rights” of medication administration
• Monitor residents’ health conditions and promptly report any changes
• Communicate effectively with physicians, charge nurses, and family members
• Support CNAs and respond to resident call lights in a timely manner
• Report any suspected abuse, neglect, or misappropriation
• Assist with daily resident care and maintain accurate documentation

Required Qualifications

• Active Wisconsin LPN License
• Current CPR/BLS Certification
• Prior experience in Long Term Care (LTC) or nursing preferred
• Strong attention to detail and communication skills
