Licensed Master Social Worker (LMSW) – Home Health

Location: Portland, OR
Assignment Length: 13 Weeks
Schedule: Day Shift | 5x8 Hours
Shift Timing: 8:00 AM – 4:30 PM
Weekly Hours: 40 Hours

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Licensed Master Social Worker (LMSW) for a Home Health travel assignment in East Portland and surrounding communities. This role is ideal for a social worker who is passionate about providing patient-centered care in a home health setting and thrives in an independent, field-based environment.

The LMSW will work collaboratively with interdisciplinary healthcare teams to support patients and families through psychosocial assessments, care coordination, counseling, and access to community resources.

Key Responsibilities

  • Conduct psychosocial assessments and evaluate patient and family needs in the home setting
  • Develop and implement individualized care plans in collaboration with healthcare teams
  • Provide counseling, emotional support, and crisis intervention services
  • Coordinate community resources and assist with access to social, financial, and healthcare services
  • Educate patients and caregivers regarding support programs and long-term care planning
  • Maintain accurate and timely documentation within electronic medical record systems
  • Participate in discharge planning and continuity of care coordination
  • Travel to patient homes within assigned coverage areas
  • Ensure compliance with agency policies, state regulations, and professional standards
  • Manage daily patient visits efficiently while meeting productivity expectations

Qualifications

  • Active Oregon LMSW license required
  • Washington LMSW license preferred or currently in process
  • Minimum 1 year of Skilled Home Health experience required
  • Oasis documentation experience required
  • Current BCLS certification required
  • Experience with EPIC charting systems preferred
  • Valid driver’s license, reliable transportation, and current auto insurance required
  • Strong communication, organizational, and time management skills
  • Ability to work independently in a field-based environment
  • COVID-19 vaccination series plus one booster required
