Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Director of Case Management to lead hospital case management operations within a respected acute care facility in North Richland Hills, TX. This leadership role is responsible for overseeing care coordination, discharge planning, regulatory compliance, departmental operations, and patient outcome initiatives.

The ideal candidate will bring strong acute care leadership experience, operational expertise, and the ability to collaborate effectively with executive leadership and interdisciplinary clinical teams.

Key Responsibilities

• Oversee daily Case Management department operations
• Lead discharge planning and care coordination initiatives
• Ensure compliance with hospital policies and regulatory standards
• Manage staffing, productivity, onboarding, and departmental resources
• Collaborate with executive leadership and medical staff
• Monitor department performance metrics and drive process improvements
• Support patient throughput and resource utilization initiatives
• Foster a collaborative and team-oriented work environment
• Provide leadership during complex case management situations

Required Qualifications

• Bachelor’s Degree required
• Master’s Degree preferred (Nursing, Healthcare Administration, or Business Administration)
• Active RN, LCSW, or LMSW license required
• Minimum 3+ years acute care hospital case management experience required
• Minimum 2+ years leadership experience in Case Management required
• Strong operational and leadership skills within acute care settings
