Job Overview

A healthcare facility is hiring a Physical Therapist for a 13-week assignment. The therapist will work with patients to improve mobility, strength, balance, and overall physical function. This role involves creating treatment plans, providing therapy sessions, and working closely with the healthcare team to support patient recovery.

Responsibilities

• Evaluate patients’ physical condition and mobility
• Create individualized treatment plans and therapy goals
• Provide therapy including:
  • Therapeutic exercises
  • Gait and transfer training
  • Mobility and balance training
  • Therapeutic modalities
• Educate patients and families on exercises and recovery techniques
• Recommend assistive devices and mobility equipment
• Monitor patient progress and update treatment plans as needed
• Maintain accurate patient documentation and therapy notes
• Work collaboratively with physicians, nurses, and rehabilitation staff
• Assist with discharge planning and continuity of care

Requirements

• Bachelor’s Degree in Physical Therapy or higher
• Current Illinois Physical Therapist license or eligibility
• Strong knowledge of rehabilitation and therapy techniques
• Good communication and documentation skills

Preferred Experience

• 1–2 years of Physical Therapy experience preferred
• Experience in rehabilitation and functional mobility training
