Job Title: Physical Therapist – Skilled Nursing Facility (SNF)
Location: Lake Havasu City, AZ
Assignment Length: 13 Weeks
Shift: Day Shift
Schedule: 5 Days/Week – 32 Hours Guaranteed

Compensation:

• Pay Rate: $45–$61/hour (based on experience)

Position Overview:

We are seeking a compassionate and skilled Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing Facility (SNF) setting. This opportunity is ideal for therapists who enjoy working with a geriatric patient population and are dedicated to improving mobility, independence, and overall quality of life through rehabilitative care.

Responsibilities:

• Evaluate and treat patients in a Skilled Nursing Facility environment
• Develop individualized treatment plans focused on mobility, balance, strength, and functional independence
• Provide therapeutic interventions and rehabilitation services tailored to patient needs
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Maintain timely, accurate, and compliant clinical documentation
• Educate patients and caregivers regarding treatment plans, safety, and home exercise programs
• Monitor patient progress and adjust treatment plans as appropriate
• Ensure compliance with therapy standards, facility policies, and healthcare regulations

Qualifications & Requirements:

• Active Arizona Physical Therapist License required at time of submission
  • Candidates who have already applied for Arizona licensure may also be considered if an estimated approval date can be provided
• Skilled Nursing Facility (SNF) or rehabilitation experience preferred
• Strong communication, interpersonal, and patient care skills
• Ability to work independently in a fast-paced clinical environment
• Strong organizational and documentation skills
