Schedule:

• Hours: 34–38 hours per week (~8.5-hour shifts)
• Days: 4 days per week
• Shift: 8:00 AM – 4:30 PM (Day Shift)

Position Overview

"Navitas Healthcare, LLC" is seeking a skilled and compassionate Registered Nurse (RN) specializing in Wound Care to provide high-quality care for patients with wound, ostomy, and continence conditions. This role involves comprehensive patient assessment, treatment planning, patient education, and collaboration with interdisciplinary teams to ensure optimal outcomes.

Key Responsibilities

• Perform comprehensive assessments focused on wound, ostomy, and continence care
• Conduct physical evaluations and develop individualized care plans
• Recommend and administer appropriate treatments and medications
• Monitor, evaluate, and document patient progress and response to treatment
• Manage complex cases, including wounds, fistulas, and ostomy care
• Collaborate with multidisciplinary healthcare teams and coordinate referrals as needed
• Educate patients, families, and staff on wound care management and prevention strategies
• Support and participate in staff education and training initiatives
• Implement preventative measures to reduce the risk of pressure injuries
• Contribute to quality improvement initiatives and clinical best practices
• Ensure compliance with patient confidentiality and regulatory standards
• Assist in the management of specialized equipment (e.g., wound care devices, specialty beds)
• Coordinate utilization of community resources to support patient care

Required Qualifications

• Minimum 2 years of Med/Surg nursing experience
• Hands-on experience in wound care and colostomy management
• Associate Degree in Nursing (ASN/ADN) from an accredited program
• Active New York State RN License (verified through NYSED)
• BLS Certification (AHA)

Required Certifications (One of the Following)

• CWOCN – Wound, Ostomy and Continence Nursing Certification
• CWON – Wound, Ostomy Nurse Certification
• CWCN – Certified Wound Care Nurse
• CWS – Certified Wound Specialist
• WCC – Wound Care Certified
