Job Description:
We are seeking an experienced RN Case Manager to support care coordination and discharge planning in an acute care hospital setting. The ideal candidate will have strong clinical knowledge, the ability to manage a high patient caseload, and experience working collaboratively with interdisciplinary teams to ensure efficient and effective patient care transitions.

Responsibilities:

• Coordinate patient care and discharge planning
• Manage a caseload of approximately 20 patients
• Collaborate with physicians, nurses, and ancillary staff
• Ensure appropriate utilization of healthcare resources
• Monitor patient progress and adjust care plans as needed
• Communicate with patients and families regarding care plans and discharge needs
• Maintain accurate documentation

Requirements:

• Active California RN license (required; must be active by start date)
• Minimum 2 years of acute care hospital Case Management experience (required)
• Experience in care coordination and discharge planning (required)
• Strong communication and organizational skills
• Ability to manage a high patient ratio
