Location: Mission Viejo, CA 92691
Duration: 13 Weeks


Schedule:

  • Shift: Day (5x8-Hour Shifts)
  • Hours: 08:00 AM – 04:30 PM
  • Weekend Requirement: Every other weekend rotation
  • Floating: May be required to affiliated facilities

Job Summary: We are seeking an experienced RN Case Manager for a contract assignment in a fast-paced acute care environment. The ideal candidate will be responsible for coordinating patient care, facilitating discharge planning, and ensuring efficient utilization of healthcare services while maintaining compliance with regulatory standards.


Key Responsibilities:

  • Coordinate patient care and manage discharge planning processes
  • Conduct case management activities and support utilization review functions (non-UR role)
  • Develop, implement, and monitor patient care plans
  • Coordinate post-acute services including SNF, DME, and transportation
  • Facilitate interdisciplinary meetings and family care discussions
  • Manage complex patient populations, including behavioral health, homelessness, and substance abuse cases
  • Collaborate with insurance providers (HMO/IPA) for care coordination and approvals
  • Ensure compliance with CMS, HIPAA, OSHA, and Joint Commission standards
  • Maintain accurate and timely documentation in Epic EMR system

Requirements:

  • Minimum 2 years of acute care Case Management experience (required)
  • Strong experience in care coordination and discharge planning
  • Experience in acute hospital setting required
  • Ability to manage a patient ratio of approximately 1:25
  • Willingness to work rotating weekends (every other weekend)
  • Flexibility to float to affiliated facilities as needed
  • California RN license required prior to start (pending license acceptable at submission)

Certifications:

  • Active RN License – California (Required)
  • BLS Certification (Required)

Education:

  • BSN or equivalent nursing degree preferred
