Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (Travel Assignment)

Location: Falmouth, MA 02540
Duration: 13 Weeks
Shift: 3:00 PM – 11:30 PM


Position Overview

The Licensed Practical Nurse (LPN) provides direct patient care to residents in a long-term care setting while supporting nursing operations and supervising nursing assistants. This role ensures high-quality, patient-centered care in compliance with clinical standards, facility policies, and regulatory requirements.


Key Responsibilities

  • Deliver direct nursing care in accordance with care plans and physician orders
  • Supervise and support daily activities of nursing assistants
  • Administer medications and treatments safely and accurately
  • Monitor, assess, and document resident conditions and responses to care
  • Transcribe and implement physician orders into medical records and care plans
  • Communicate resident status updates with physicians and interdisciplinary teams
  • Perform clinical procedures including catheterization, tube feedings, suctioning, wound care, and specimen collection
  • Record vital signs and maintain accurate clinical documentation
  • Support resident admissions, transfers, and discharges with required documentation
  • Ensure proper inventory and availability of medications, supplies, and equipment
  • Provide education and support to residents and family members
  • Participate in facility surveys, inspections, and quality improvement initiatives
  • Maintain confidentiality and uphold resident rights at all times
  • Collaborate with the healthcare team to ensure coordinated care delivery
  • Perform additional duties as assigned

Required Qualifications

  • Graduate of an approved LPN nursing program
  • Active, unrestricted LPN license in Massachusetts
  • Current BLS / CPR / AED certification
  • Strong knowledge of long-term care clinical practices and regulations
  • Ability to supervise staff and make independent clinical decisions
  • Excellent communication and interpersonal skills
  • Ability to handle complex and sensitive situations with professionalism

Experience

  • Prior experience in long-term care or skilled nursing preferred
  • Previous supervisory experience with nursing assistants strongly preferred
