Position Overview:

"Navitas Healthcare, LLC" is seeking a licensed Physical Therapist to provide home health therapy services in West Portland, OR, and surrounding areas. This role is ideal for skilled PTs interested in patient-focused, in-home care with flexible scheduling in a supportive home health environment.

Key Responsibilities:

• Provide skilled PT services in patients’ homes (up to 6 visits per day).
• Develop and implement individualized care plans to improve functional independence.
• Conduct patient assessments, monitor progress, and adjust therapy interventions as needed.
• Document all patient visits using EPIC/Oasis software.
• Maintain patient safety and adhere to infection control and home health regulations.
• Coordinate care with nurses, physicians, and other multidisciplinary team members.

Requirements:

• Active Oregon PT license.
• 1 year of skilled home health experience required. First-timers accepted.
• BCLS certification.
• Reliable transportation for home visits.
• Ability to work independently in a field-based environment.
• Oasis and EPIC experience preferred.

Schedule:

• Monday – Friday, 08:00 AM – 04:30 PM
• 5x8-Hour shift
• No weekends required
• No block schedule accommodation
