Hours: 40 hours/week

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) – Case Manager to support care coordination within a fast-paced acute care environment. This role is responsible for managing patient care plans, ensuring appropriate utilization of healthcare services, and facilitating safe and effective transitions of care.

Key Responsibilities

• Manage case coordination for an assigned patient caseload (approx. 1:25 ratio)
• Develop, implement, and evaluate individualized patient care plans
• Coordinate discharge planning and post-acute care services
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Ensure appropriate utilization of healthcare resources and services
• Maintain accurate and timely clinical documentation
• Communicate effectively with patients and families regarding care plans
• Monitor patient progress and adjust care plans as needed
• Float to affiliated facility as required
• Participate in weekend rotation schedule

Required Qualifications

• Minimum 2 years of acute care case management experience
• Strong background in care coordination and discharge planning
• Ability to manage high-volume caseloads efficiently
• Excellent communication, organizational, and critical thinking skills
• Prior travel experience required (first-time travelers will not be considered)

Education

• Graduate of an accredited Registered Nursing program

Certifications & Licensure

• Active California RN License (must be active prior to start date; pending accepted)
• BLS Certification (per facility requirements)
