Job Title: Licensed Practical Nurse (LPN) – Long-Term Care & Rehab

Location: Rochester, NY 14612
Duration: 8-Week Contract
Schedule: 5x8-Hour Shifts (40 hours/week)
Shift: Primarily Evenings (Flexibility for Nights Required)


Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to join a Long-Term Care and Rehabilitation setting. This role involves delivering high-quality patient care in a fast-paced, team-oriented environment, with flexibility to work across multiple units.


Key Responsibilities

  • Provide direct patient care in long-term care (LTC) and rehabilitation settings
  • Administer medications and perform prescribed treatments
  • Monitor patient conditions and document accurately using Epic EMR
  • Perform blood glucose monitoring and other routine clinical procedures
  • Assist patients with activities of daily living (ADLs)
  • Collaborate with interdisciplinary healthcare teams
  • Float between units as required to support patient care needs

Required Qualifications

  • Active New York Licensed Practical Nurse (LPN) License
  • BLS (Basic Life Support) Certification
  • Minimum 1 year of LPN experience in a long-term care setting
  • Experience in Skilled Nursing Facility (SNF) or nursing home environment
  • Experience with Epic EMR system
  • Willingness to float across 3 units

Additional Information

  • First-time travelers are welcome
  • Weekend Requirement: Every other weekend (8 weekend shifts over 8 weeks)
  • Local Candidates: Must reside at least 75 miles from the facility
  • COVID-19 Vaccine: Not required
