Job Title: Physical Therapist (Home Health – Outpatient)

Location: Medford, OR (and surrounding areas)
Duration: 13 Weeks Contract
Shift: Day Shift (08:00 AM – 04:30 PM)
Schedule: 5x8-hour shifts

Position Overview:

We are seeking a compassionate and independent Physical Therapist (PT) to provide home health services in the Medford, OR area. This role offers the opportunity to work one-on-one with patients in a home setting, focusing on improving mobility, function, and overall quality of life.

Key Responsibilities:

• Provide in-home physical therapy services to patients
• Evaluate, develop, and implement individualized treatment plans
• Monitor patient progress and adjust therapy as needed
• Maintain accurate and timely documentation in EMR systems
• Educate patients and caregivers on exercises, safety, and recovery plans
• Collaborate with interdisciplinary healthcare teams
• Manage daily patient visits and travel efficiently within assigned areas

Required Qualifications:

• Active Physical Therapist license (or eligibility)
• BLS (Basic Life Support) certification required
• Ability to work independently in a home health setting
• Reliable transportation for patient visits

Preferred Qualifications:

• Experience with OASIS documentation
• Familiarity with EPIC or similar EMR systems
• Previous home health experience (not required; candidates open to learning home health are welcome)
