Day Shift: 6:00 AM – 6:30 PM
Night Shift: 6:30 PM – 6:00 AM

Position Overview

"Navitas Healthcare, LLC" is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a structured healthcare setting. The LPN will support residents through medication administration, treatments, and coordination with the healthcare team while ensuring compliance with state and facility standards.

Key Responsibilities

Administer medications (oral, enteral, parenteral, etc.)
Perform treatments (wound care, trach care, catheter care, dressing changes)
Monitor patient response and report changes in condition
Maintain medication and supply availability
Participate in shift handoffs and patient reporting
Communicate with residents, families, and care teams
Conduct initial assessments for new admissions
Assist with MDS, care planning, and reviews
Maintain accurate documentation of patient care
Participate in QAPI and quality improvement initiatives

Required Qualifications

Active Arizona LPN License
Strong clinical judgment and emergency response ability
Ability to assess, document, and report patient conditions
Strong communication and teamwork skills

Preferred Qualifications

Experience in geriatric or long-term care settings
Familiarity with care planning and MDS processes
