Shift: Day Shift (5x8) | 07:30 AM – 04:00 PM
Schedule: Every other weekend required | Floating between hospital units and campuses

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) with acute care background to join a hospital-based Case Management team. The role focuses on delivering comprehensive psychosocial support, discharge planning, and care coordination across multiple inpatient units and campuses.

Key Responsibilities

• Provide case management and care coordination for patients across multiple hospital units
• Conduct psychosocial assessments and develop effective discharge plans
• Coordinate safe and timely transitions of care
• Collaborate with interdisciplinary teams including physicians, nurses, and therapists
• Support patients and families with counseling and community resource linkage
• Maintain accurate and timely documentation in EPIC EHR system
• Float between campuses and units as operationally required (locations within close proximity)
• Participate in an every-other-weekend rotation (guaranteed 72 hours per two-week period)

Requirements

• Active Washington State LMSW license (required)
• Minimum 1 year of acute care social work experience
• Experience in discharge planning and care coordination
• Strong background in hospital-based case management (Med/Surg, ED, Long Length of Stay preferred)
• Current BLS certification required
• Ability to work across multiple units and campuses as needed
