Position Title: Registered Nurse (RN) – Home Health / Hospice

Location: Gunnison, Colorado
Job Type: Contract
Shift: Day Shift
Schedule: 36 hours/week + 1–2 nights on-call per week + 1–2 weekends per month (holiday coverage required)
Guaranteed Hours: 36 hours/week

Position Overview

"Navitas Healthcare, LLC" is seeking a Registered Nurse (RN) to provide skilled nursing care in home health, hospice, and palliative care settings. The RN will be responsible for delivering patient-centered care in home environments, coordinating with physicians and interdisciplinary teams, and managing individualized care plans to support patient comfort, safety, and quality of life.

Key Responsibilities

• Perform initial patient assessments and ongoing reassessments
• Develop, implement, and update individualized care plans
• Provide skilled nursing care in home health and hospice settings
• Document clinical notes and patient progress in Epic EMR
• Coordinate care with physicians and interdisciplinary team members
• Educate patients and families on care plans, medications, and disease management
• Supervise and support other nursing staff as needed
• Participate in quality improvement initiatives and in-service training
• Provide end-of-life care support with dignity and compassion

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum 3 years of nursing experience
• BLS/BCLS certification (required)
• Prior experience in home health or hospice care (required)
• Ability to work independently in field-based settings
• Valid driver’s license and reliable transportation
• Strong communication, assessment, and case management skills
