Position Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility
Location: Wray, Colorado
Assignment Duration: 10 Weeks
Shift Type: Evening / Night Shift
Schedule: 5 Days per Week
Shift Details: Variable (8-hour or 12-hour shifts)
Weekly Hours: Approximately 40 Hours
Job Summary:
"Navitas Healthcare, LLC" is a dedicated and compassionate Licensed Practical Nurse (LPN) to provide direct patient care in a Skilled Nursing Facility (SNF). The LPN will support daily nursing operations, assist in implementing individualized care plans, and collaborate with RNs and the interdisciplinary team to ensure safe, efficient, and high-quality resident care.
Key Responsibilities:
Provide direct patient care in accordance with resident care plans and facility policies
Assist in implementing and evaluating individualized resident care under RN supervision
Administer medications and treatments within scope of practice and facility guidelines
Monitor resident conditions and report changes promptly to RN or physician
Collaborate with RNs, physicians, therapists, and interdisciplinary teams
Assist with patient rounds and address resident and family concerns
Maintain accurate and compliant documentation of care provided
Support CNAs and junior staff under RN supervision
Participate in quality improvement initiatives and audits as directed
Ensure compliance with SNF/LTC regulations and facility standards
Qualifications:
Completion of an accredited Practical Nursing program
Active Licensed Practical Nurse (LPN) license required
Basic Life Support (BLS) certification required
COVID-19 vaccination required (medical exemptions only)
ACLS preferred
IV certification preferred
0–2 years of LPN experience preferred
Skilled Nursing Facility or Long-Term Care experience strongly preferred
Skills & Competencies:
Strong clinical skills in SNF/LTC settings
Ability to support and guide CNAs under RN supervision
Knowledge of long-term care regulations and compliance standards
Strong communication and teamwork skills
Excellent organizational and multitasking ability
Attention to detail in documentation and medication administration
Compassionate, resident-focused approach to care
