Job Title: Physical Therapist – Home Health

Job Type: Contract (13 Weeks)

Work Schedule: Day Shift – 5x8 Hours (8:00 AM – 4:30 PM)

Location: Medford, OR

Job Summary:

We are seeking an experienced Physical Therapist (PT) for a home health setting. The ideal candidate will provide one-on-one patient care, focusing on improving mobility, strength, and overall functional independence in patients’ homes.

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services in a home setting
• Monitor patient progress and adjust care plans as needed
• Complete accurate and timely documentation using EMR systems
• Educate patients and caregivers on exercises and safety techniques
• Manage daily caseload efficiently while meeting productivity standards
• Travel between patient homes within the assigned service area

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent
• Active state PT license (or eligibility)
• Minimum 1 year of skilled home health experience required
• Experience with OASIS documentation required
• Strong communication and organizational skills
• Ability to work independently in a home health environment

Certifications:

• BLS certification required
