Job Title: Occupational Therapist – Home Health

Job Type: Contract (13 Weeks)

Work Schedule: Day Shift – 5x8 Hours (8:00 AM – 4:30 PM)

Location: Medford, OR

Job Summary:

We are seeking an experienced Occupational Therapist (OT) for a home health setting. The ideal candidate will provide one-on-one patient care, helping individuals improve daily living skills and overall independence in their home environments.

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans
• Provide in-home occupational therapy services
• Monitor patient progress and adjust treatment plans as needed
• Complete accurate and timely documentation using EMR systems
• Educate patients and caregivers on therapy techniques and home safety
• Manage a daily caseload while maintaining productivity standards
• Travel between patient homes within the assigned service area

Patient Care Expectations:

• 1:1 patient ratio
• Up to 6 patient visits per day
• Productivity expectation: ~30 points per week

Qualifications:

• Active Occupational Therapist (OT) license (or eligibility)
• Minimum 1 year of skilled home health experience required
• Experience with OASIS documentation required
• Strong communication and time management skills
• Ability to work independently in a home health environment

Certifications:

• BLS certification required
