Employment Type: Full-Time (40 hours/week)
Shift: Evenings (8-hour shifts)

Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) facility. The LPN will provide direct patient care to residents, ensuring their safety, comfort, and overall well-being. This role works under the supervision of a Registered Nurse (RN) and collaborates closely with the healthcare team to deliver high-quality, patient-centered care.

Key Responsibilities

• Provide direct nursing care to long-term care residents
• Administer medications and treatments as prescribed
• Monitor, record, and report vital signs and patient conditions
• Assist residents with Activities of Daily Living (ADLs)
• Perform wound care and other clinical nursing procedures
• Maintain accurate and timely patient documentation
• Report any changes in patient condition to RN or physician
• Follow infection control and facility safety protocols
• Support implementation and updates of individualized care plans
• Respond promptly to resident needs and emergency situations

Required Qualifications

Education:

• Diploma or certification from an accredited Practical Nursing program (or equivalent educational qualification)

Licensure/Certification:

• Active LPN/LVN license (state of practice)
• Current Basic Life Support (BLS) certification required

Experience:

• Minimum 1 year of LPN experience in Long-Term Care or Skilled Nursing Facility preferred
• Experience with Electronic Medical Records (EMR) systems preferred
• Ability to work flexible schedules, including weekends and holidays
