Position Title: Licensed Practical Nurse (LPN) – Long Term Care (LTC)
Location: Rochester, NY 14612
Weekly Hours: 40 – 42.5 hours
Schedule: Evening Shift – 5x8 hours (3:00 PM – 11:30 PM) | No block scheduling
Contract Duration: 8 weeks

Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a Long-Term Care (LTC) and rehabilitation setting. This role involves delivering direct nursing care, administering treatments, and supporting residents in maintaining comfort and overall well-being within a skilled nursing facility.

Key Responsibilities

• Provide direct nursing care to LTC and rehabilitation patients
• Administer medications and monitor patient responses
• Perform blood glucose monitoring and other clinical procedures
• Monitor, assess, and document patient conditions and vital signs
• Assist in the development and implementation of care plans
• Collaborate with Registered Nurses (RNs) and interdisciplinary teams
• Ensure patient safety, comfort, and quality care delivery
• Maintain accurate documentation using EPIC
• Float between units as required
• Respond promptly to patient needs and emergencies

Required Qualifications

Education:

• Diploma or certification from an accredited Practical Nursing program

Licensure / Certification:

• Active New York LPN License – Required
• Basic Life Support (BLS) Certification – Required

Experience & Requirements

• Minimum 1 year of LPN experience in a Long-Term Care (LTC) setting
• First-time travelers are welcome
• Must be willing to float across all units
• Must reside more than 75 miles from the facility (no local candidates)
• Experience with EPIC charting required
• Availability for every other weekend (total of 8 weekend shifts)
