Guaranteed Hours: 35 hours per week

Job Summary

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join an acute care hospital setting. This role involves delivering comprehensive social work services to patients in inpatient medicine and surgical units, with a strong focus on care coordination, discharge planning, and psychosocial support.

Key Responsibilities

• Conduct psychosocial assessments for patients and their families
• Provide emotional support and counseling as needed
• Coordinate discharge planning and facilitate connections to community resources
• Collaborate with interdisciplinary teams including physicians, nurses, and case managers
• Support patients within inpatient medicine and surgical (med-surg) units
• Maintain accurate and timely documentation in compliance with healthcare standards
• Advocate for patient needs to ensure continuity and quality of care

Required Qualifications

• Master’s Degree in Social Work (MSW)
• Active LMSW license issued by New York State Education Department (NYSED)
• Minimum of 1 year of experience as an LMSW
• Prior experience in acute care or inpatient settings (medicine/surgery/med-surg) is required
• Strong communication, organizational, and teamwork skills
