Job Title: Physical Therapist

Location: Ladson, SC
Duration: 13 Weeks
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Employment Type: Contract

Job Summary:

We are seeking a skilled Physical Therapist to provide high-quality rehabilitative care in a home health setting. The Physical Therapist will evaluate patients, develop treatment plans, and deliver therapeutic interventions aimed at restoring mobility, strength, and overall functional independence.

Responsibilities:

  • Evaluate patients and develop individualized physical therapy treatment plans
  • Provide hands-on therapeutic care in accordance with established plans and standards of practice
  • Improve patient mobility, strength, safety, and functional abilities
  • Document all patient care, progress, and coordination of services accurately in the EMR
  • Supervise Physical Therapy Assistants (PTAs) and Home Health Aides to ensure treatment goals are met
  • Coordinate care with interdisciplinary teams, families, and external providers
  • Utilize therapeutic exercises, modalities, and assistive devices to support rehabilitation
  • Educate patients and caregivers on treatment plans, home exercises, and safety measures
  • Maintain compliance with clinical, safety, and regulatory standards

Required Qualifications:

  • Minimum 1 year of experience as a Physical Therapist
  • Home Health experience required
  • Current Physical Therapist license in the state of South Carolina
  • Current BLS Certification (American Heart Association)
  • Valid driver’s license and ability to travel for patient visits

Education:

  • Graduate of an accredited Physical Therapy program

Key Skills:

  • Strong knowledge of rehabilitative therapy techniques and modalities
  • Experience with orthotic, prosthetic, and assistive devices
  • Excellent organizational, communication, and time management skills
  • Ability to work independently in a home health environment
