Job Title: Travel LPN / LVN – Long Term Care
Location: Utica, NY

Assignment Details:

• Duration: 13 Weeks
• Shift: Day Shift, 6:30 AM – 2:30 PM
• Hours: 37.5 hours/week (guaranteed)
• Schedule: Includes every other weekend
• Floating: Required across all units within the building

Job Overview:
"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to join a long-term care facility in Utica, NY. This role is ideal for candidates experienced in diverse patient populations, including memory care and orthopedic patients. Both travel and local candidates are encouraged to apply.

Key Responsibilities:

• Provide direct patient care in a long-term care setting
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and progress
• Assist patients with daily living activities including hygiene, mobility, and feeding
• Deliver care to patients with conditions such as MS, orthopedic needs, dementia, and memory care
• Maintain accurate and timely documentation using EMR (PointClickCare)
• Collaborate with RNs, physicians, and interdisciplinary team members
• Ensure compliance with facility policies, safety standards, and infection control protocols
• Respond promptly to patient needs and changes in condition
• Float between units as required to support patient care needs

Qualifications:

• Active New York State LPN license required
• Minimum 1+ year of LPN experience in long-term care or a similar setting
• Current BLS (Basic Life Support) certification required
• Experience with geriatric and memory care patients preferred
• Familiarity with PointClickCare EMR system is a plus
• Strong clinical, communication, and organizational skills
• Ability to adapt and work across multiple units
• Compassionate, patient-focused, and team-oriented

Additional Information:

• Travel Requirement: Candidates residing outside a 75-mile radius qualify for travel rates
• Local Candidates: Accepted with adjusted pay rate for those within 75 miles
