Job Title: Licensed Practical Nurse (LPN) – Long-Term Care
Location: Falmouth, MA 02540
Pay Rate: $50.00 – $60.00 per hour
Schedule: Full-Time | 40 Hours per Week

Shift Details:

  • Day Shift: 7:00 AM – 3:30 PM
  • Evening Shift: 3:00 PM – 11:30 PM
  • Rotational weekends and holidays required

Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) facility. In this role, you will provide direct patient care, support resident well-being, and supervise nursing assistants while ensuring compliance with healthcare regulations. This position requires strong clinical skills, attention to detail, and the ability to work collaboratively within a healthcare team.


Key Responsibilities

  • Deliver direct nursing care in accordance with care plans and physician orders
  • Supervise and guide nursing assistants in daily patient care activities
  • Administer medications and treatments safely and accurately
  • Monitor, assess, and document resident conditions and responses to treatment
  • Maintain accurate and timely medical records, including admissions, transfers, and discharges
  • Communicate effectively with physicians and interdisciplinary teams
  • Participate in care planning and physician rounds
  • Perform clinical procedures such as wound care, catheterization, tube feeding, and specimen collection
  • Record vital signs and report changes in patient condition
  • Ensure adequate medical supplies and equipment availability
  • Educate residents and families on health-related matters
  • Support compliance with all regulatory standards and facility policies
  • Assist with new resident orientation and provide exceptional patient-centered care

Required Qualifications

Education:

  • Completion of an accredited LPN program or Nursing Degree

Licensure/Certifications:

  • Active, unencumbered LPN license (Massachusetts)
  • BLS / CPR / AED certification

Experience & Skills:

  • Knowledge of long-term care practices and healthcare regulations
  • Strong leadership and supervisory skills
  • Excellent communication and interpersonal abilities
  • Ability to make sound clinical decisions independently
  • Proficient in documentation and patient record management
  • Team-oriented with a compassionate, patient-first approach
