Position Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF)
Location: Keene, NH 03431
Pay Rate: $25 – $35/hour (Based on shift and experience)

Schedule: 3 – 4 days per week
Shift Type: Day, Evening, and Night shifts available
Hours per Shift: 8 or 12-hour shifts
Weekly Hours: Approximately 36 – 40 hours


Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide quality patient care in a Skilled Nursing Facility (SNF) setting. The LPN will work under the supervision of a Registered Nurse (RN) or physician, supporting patient care, administering medications, and ensuring a safe and comfortable environment for residents.


Key Responsibilities

  • Deliver direct patient care in accordance with individualized care plans

  • Administer medications and treatments as prescribed

  • Monitor vital signs and report any changes to the RN or physician

  • Assist with wound care, dressing changes, and basic nursing procedures

  • Support residents with activities of daily living (ADLs)

  • Collaborate with nursing staff and interdisciplinary team members

  • Maintain accurate and timely patient documentation

  • Follow infection control and safety protocols

  • Respond promptly to patient needs

  • Participate in care planning and team meetings


Required Qualifications

Education:

  • Diploma or Certificate in Practical Nursing from an accredited institution (preferred)

Licensure/Certification:

  • Active LPN License (required)

  • Basic Life Support (BLS) certification (required)

  • IV Certification (preferred)

Experience:

  • New graduates are welcome; training provided

  • Minimum 1 year of nursing experience (preferred)

  • Experience in SNF or long-term care setting (preferred)
