Position Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF)
Location: Keene, NH 03431
Pay Rate: $25 – $35/hour (Based on shift and experience)
Schedule: 3 – 4 days per week
Shift Type: Day, Evening, and Night shifts available
Hours per Shift: 8 or 12-hour shifts
Weekly Hours: Approximately 36 – 40 hours
"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide quality patient care in a Skilled Nursing Facility (SNF) setting. The LPN will work under the supervision of a Registered Nurse (RN) or physician, supporting patient care, administering medications, and ensuring a safe and comfortable environment for residents.
Deliver direct patient care in accordance with individualized care plans
Administer medications and treatments as prescribed
Monitor vital signs and report any changes to the RN or physician
Assist with wound care, dressing changes, and basic nursing procedures
Support residents with activities of daily living (ADLs)
Collaborate with nursing staff and interdisciplinary team members
Maintain accurate and timely patient documentation
Follow infection control and safety protocols
Respond promptly to patient needs
Participate in care planning and team meetings
Education:
Diploma or Certificate in Practical Nursing from an accredited institution (preferred)
Licensure/Certification:
Active LPN License (required)
Basic Life Support (BLS) certification (required)
IV Certification (preferred)
Experience:
New graduates are welcome; training provided
Minimum 1 year of nursing experience (preferred)
Experience in SNF or long-term care setting (preferred)
Strong clinical and patient care skills
Knowledge of standard nursing practices and procedures
Excellent communication and interpersonal skills
Ability to work effectively in a team environment
Strong organizational and time management abilities
Attention to detail in patient care and documentation
Ability to multitask and prioritize tasks efficiently
Compassionate, patient-focused approach
