Location - Phoenix, AZ
Shift: Day Shift – 12 Hours (6:00 AM – 6:30 PM)
Hours: 30 Hours per Week
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care/geriatric setting. The LPN will be responsible for administering medications, performing treatments, and supporting residents’ overall health while working closely with an interdisciplinary team.
Administer medications via oral, enteral, parenteral, and other routes as prescribed
Perform treatments including:
Wound care and sterile dressing changes
Tracheostomy, gastrostomy, and jejunostomy care
Catheter care and postural drainage procedures
Monitor and document patient responses to medications and treatments
Complete initial admission assessments for new residents
Assist with MDS assessments, care plans, and quarterly reviews
Maintain accurate clinical and administrative documentation
Participate in shift reports and interdisciplinary communication
Ensure availability of medical supplies and medications
Communicate effectively with residents, families, and care teams
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Attend and contribute to staff meetings
Active LPN License (State-specific)
Experience in long-term care or geriatric nursing preferred
Knowledge of medication administration and treatment protocols
Understanding of state and federal nursing regulations
Strong communication and interpersonal skills
