Location- Phoenix, AZ
Shift: Night Shift – 12 Hours (6:30 PM – 6:00 AM) | 40 Hours/Week
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a long-term care/geriatric setting. The LPN will be responsible for administering medications, performing treatments, and supporting residents’ overall health and well-being while collaborating with an interdisciplinary care team.
Administer medications via oral, enteral, parenteral, and other routes as prescribed
Perform clinical treatments including:
Wound care and sterile dressing changes
Tracheostomy, gastrostomy, and jejunostomy care
Catheter care and postural drainage procedures
Monitor and document resident responses to treatments and medications
Conduct initial nursing assessments for new admissions
Assist in developing and updating care plans and MDS assessments
Maintain accurate clinical and administrative documentation
Participate in shift reports and interdisciplinary communication
Ensure availability of medical supplies and medications
Communicate effectively with residents, families, and healthcare staff
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Attend and contribute to staff meetings and team discussions
Active LPN License (State-specific)
Experience in long-term care or geriatric nursing preferred
Strong knowledge of medication administration and treatment protocols
Understanding of state and federal nursing regulations
Excellent communication and interpersonal skills
