Job Title: Registered Nurse – Home Health / Hospice

Location: Gunnison, CO
Contract Duration: 13 Weeks
Shift: Day Shift with Call Requirements

  • On-call: 1–2 nights per week and 1–2 weekends per month


Compensation:

  • Weekly Gross Pay: $1,800 – $1,900 (based on experience)


Job Overview:

We are seeking an experienced Registered Nurse (RN) to provide compassionate, high-quality care to patients in home health, hospice, and palliative care settings. This role involves direct patient care, case management, and coordination of services to ensure optimal outcomes in a community-based environment.


Key Responsibilities:

  • Provide skilled nursing care to home health, hospice, and palliative care patients

  • Conduct initial patient assessments and ongoing reassessments

  • Develop, implement, and update individualized Plans of Care

  • Perform case management and coordinate multidisciplinary services

  • Administer specialized nursing treatments and procedures

  • Monitor patient conditions and communicate changes to physicians and care teams

  • Educate patients and families on disease management and care plans

  • Maintain accurate clinical documentation and progress notes

  • Supervise and support other nursing staff as needed


Required Certifications & Licensure:

  • Active Registered Nurse (RN) License (Colorado or Compact)

  • Basic Life Support (BLS)

  • Colorado Adult Protective Services (CAPS) check required


Required Experience:

  • Minimum 3 years of nursing experience

  • Prior experience in Home Health and/or Hospice care (required)


Key Skills:

  • Strong patient assessment and clinical decision-making abilities

  • Experience with case management and care coordination

  • Ability to work independently in a home-based care environment

  • Excellent communication and patient education skills


Work Environment:

  • Community-based care setting

  • Requires travel to patient homes

  • On-call responsibilities as part of the role
