Position: RN – Home Health
Location: South Dennis, MA 02660
Duration: 13 Weeks
Schedule: 40 hours/week, Monday–Friday
Pay Rate: $58 – $62.50 per hour
Position Summary:
Provides comprehensive home health nursing care, including assessment, planning, implementation, and evaluation of patient care to ensure safety, quality, and adherence to care objectives.
Key Responsibilities:
Conduct comprehensive nursing assessments upon patient admission, including physiological, psychological, and environmental factors.
Develop, implement, and revise individualized plans of care based on assessment findings.
Maintain annual HHRG values at the national standard and utilize them to determine visit frequency and interdisciplinary care needs.
Participate in interdisciplinary case conferences and evaluate effectiveness of care plans.
Supervise and document Home Health Aide plan of care every 14 days.
Notify providers of significant changes in patient condition and clarify MD orders as needed.
Complete all admission, discharge, and ongoing documentation accurately and timely.
Refer patients to community resources as appropriate.
Plan and coordinate patient discharges and document accordingly.
Perform other duties as assigned.
Qualifications:
Current RN license in Massachusetts.
Successful completion of pre-hire medication test.
Minimum one (1) year of nursing experience (or 6 months with BSN).
CPR certification required.
Home care, IV experience, computer skills, and BSN preferred.
