Schedule: Rotating Shift – 40 hours per week
Start time: 6:30 AM or 8:00 AM (Monday – Friday)
Rotating Weekends: 7:00 AM – 12:00 PM
Guaranteed Hours: 40 hours/week
Pay Rate: $70 – $78 per hour
A healthcare facility in Glenwood Springs, Colorado is seeking a Licensed Pharmacist to support hospital pharmacy operations. The pharmacist will be responsible for ensuring safe and effective medication therapy, preparing and dispensing medications, and providing clinical consultation to healthcare providers and patients.
The role involves working collaboratively with physicians, nurses, and other healthcare professionals to ensure optimal patient care and medication management.
Interpret medication orders and accurately enter them into computerized patient medication profiles.
Prepare, compound, and dispense pharmaceuticals including:
IV admixtures
Chemotherapy medications
Parenteral nutrition products
Ensure compliance with federal, state, and local pharmacy regulations.
Dispense and maintain records for controlled substances in accordance with legal requirements.
Monitor medication therapy for:
Drug interactions
Contraindications
Allergies
Appropriate drug selection and dosing
Perform pharmacokinetic dosing according to hospital protocols.
Review and interpret culture and sensitivity (C&S) data to assess antibiotic appropriateness.
Extract and interpret relevant information from patient medical records.
Identify and report adverse drug reactions.
Support formulary management by minimizing non-formulary medications and promoting therapeutic substitutions.
Provide clinical consultation and therapeutic recommendations to physicians and medical staff.
Offer drug information and counseling to patients and families when needed.
Document clinical interventions and pharmacy activities accurately.
Participate in quality improvement initiatives and medication review programs.
Maintain departmental logs, records, and documentation as required.
Assist with training, orientation, and educational programs for pharmacy and healthcare staff.
Serve as a member of the Code Response Team when required.
Active Colorado Pharmacist (PHA) License in good standing.
Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy (BS Pharm) or equivalent.
BLS Certification – American Heart Association (AHA)
ACLS Certification – American Heart Association (AHA)
