Salary Range: $109,762 – $164,570 annually (Based on Experience)
Annual Incentive Bonus: Up to 17.5%

A leading acute care hospital in Bradenton, FL is seeking an experienced Director of Case Management to lead and oversee all case management operations within the facility. This leadership role is responsible for managing care coordination, utilization review, and discharge planning activities while ensuring compliance with hospital policies and regulatory standards.
The Director will collaborate closely with interdisciplinary teams and executive leadership to ensure high-quality patient care, efficient resource utilization, and seamless transitions of care across the continuum.
Provide leadership and oversight for daily case management operations including care coordination, utilization review, and discharge planning
Manage departmental operations including staffing, onboarding, competencies, and performance management
Ensure appropriate allocation of resources to meet productivity, quality, and operational goals
Maintain compliance with hospital policies, regulatory requirements, and care coordination standards
Monitor departmental performance through data analysis and lead performance improvement initiatives
Promote effective interdisciplinary collaboration to ensure timely patient care transitions
Partner with executive leadership, including the Chief Financial Officer, Chief Medical Officer, and Chief Nursing Officer, as well as division case management leaders to support operational and financial objectives
Oversee documentation quality, discharge planning processes, and clinical resource utilization
Develop strong relationships with internal departments and external agencies to ensure continuity of care
Bachelor’s Degree required
Active professional license: RN, LCSW, or LMSW
Minimum 3+ years of acute care hospital case management experience
Minimum 2+ years of case management leadership experience
Master’s Degree in Nursing, Healthcare Administration, or Business Administration
Strong knowledge of hospital case management operations, utilization review, and care coordination best practices
