Position Title: RN Case Manager
Location: Hyannis, MA 02601
Job Type: Travel Contract
Duration: 13 Weeks
Pay Rate: $80 – $92 per hour (Based on Experience)
Schedule: 40 Hours per Week
Shifts Available: Day, Evening, and Night
"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager to coordinate patient care, discharge planning, and utilization management within a hospital setting. The RN Case Manager works closely with physicians, nurses, and interdisciplinary teams to ensure efficient patient flow, safe care transitions, and quality patient outcomes.
Care Coordination & Case Management
Coordinate patient care plans and discharge planning
Collaborate with physicians, nurses, and social workers on treatment plans
Identify high-risk patients and coordinate appropriate resources
Ensure safe and timely patient transitions of care
Utilization Review
Monitor patient care services to ensure compliance with utilization standards
Review clinical documentation and patient status
Ensure services meet CMS and healthcare regulatory guidelines
Clinical Rounding & Patient Support
Conduct unit rounds to identify discharge barriers
Assist clinical teams in resolving patient care challenges
Support interdisciplinary care coordination
Program Support & Education
Assist with training and education related to case management processes
Support orientation of new case managers and clinical staff
Contribute to development of care coordination programs
Quality & Process Improvement
Track case management program performance and compliance
Assist with reporting and improvement initiatives
Maintain knowledge of CMS regulations and healthcare standards
Education
Bachelor’s Degree in Nursing (BSN) required
Master’s Degree preferred
Licensure
Active Massachusetts RN License
Certifications (Preferred)
Certified Case Manager (CCM)
Clinical Documentation Integrity Certification
Experience
Minimum 5 years of case management experience in acute care
Experience with utilization management and clinical documentation review
Process improvement experience preferred
Strong knowledge of CMS regulations and healthcare compliance
Excellent care coordination and discharge planning skills
Strong communication and organizational abilities
Proficiency with electronic medical records (EMR) and healthcare software
Ability to collaborate effectively with interdisciplinary healthcare teams
