Job Title: Licensed Practical Nurse (LPN)
Location: Paonia, CO 81428
Shift: Variable – Nights
Schedule: 12-Hour Shifts | 36 Hours per Week
Pay Rate: $35 – $37 per hour
"Navitas Healthcare, LLC" is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. The LPN will support resident health, safety, and comfort while working closely with an interdisciplinary healthcare team to ensure the best possible patient outcomes.
Provide direct nursing care to residents in a long-term care environment.
Administer medications and treatments as prescribed by physicians.
Monitor and record vital signs, symptoms, and patient progress.
Assist with wound care, catheter care, and other clinical procedures.
Maintain accurate and timely patient documentation in medical records.
Communicate effectively with healthcare team members, residents, and families regarding care plans and patient status.
Follow infection control standards and safety protocols.
Assist residents with activities of daily living while promoting dignity and independence.
Active Licensed Practical Nurse (LPN) license in Colorado.
BLS (Basic Life Support) Certification.
Minimum 1 year of recent experience in Long-Term Care (LTC) or Skilled Nursing Facility preferred.
Strong knowledge of patient care standards, medication administration, and clinical procedures.
Ability to work night shifts and 12-hour schedules.
