Registered Nurse (RN) – Home Health

Shift: Day Shift
Pay Rate: $58 – $62.50 per hour
"Navitas Healthcare, LLC" is seeking a dedicated Home Health Registered Nurse (RN) to provide comprehensive, patient-centered care in the home setting. The RN will perform detailed nursing assessments, develop individualized care plans, and deliver skilled nursing services to ensure high-quality outcomes and patient safety.
Conduct comprehensive admission assessments, including physiological, psychosocial, environmental, and health history evaluations.
Develop, implement, and revise individualized plans of care based on assessment findings and case management objectives.
Maintain annual HHRG values at national standards and utilize data to determine appropriate visit frequency and interdisciplinary services.
Monitor and ensure patient outcomes meet national benchmarks.
Evaluate patient needs and provide skilled nursing interventions with thorough, accurate documentation.
Participate in interdisciplinary case conferences to assess and improve care effectiveness.
Provide instruction and documented supervision of Home Health Aide care plans every 14 days.
Communicate significant changes in patient condition to physicians and care team members promptly.
Obtain and clarify physician orders for all services delivered.
Complete admissions, visit notes, and discharge documentation in a timely and accurate manner.
Plan and coordinate patient discharges, including communication with patients, caregivers, and physicians.
Refer patients to appropriate community resources as needed.
Perform additional duties as assigned.
Current Registered Nurse (RN) license in Massachusetts.
CPR certification required.
Minimum one (1) year of nursing experience required, or six (6) months with a BSN.
Successful completion of pre-employment medication exam.
Home Health and IV therapy experience preferred.
Strong documentation and computer skills preferred.
BSN preferred.
