Job Title: Director of Case Management

Salary Range: $135,004 – $202,555 (based on years of experience)
Shift: 7–10 Hour Shifts
A large, high-acuity acute care hospital in Las Vegas is seeking an experienced Director of Case Management to lead and oversee all case management operations across the facility. This executive leadership role is responsible for driving strategic initiatives, ensuring regulatory compliance, and integrating patient care coordination and discharge planning processes to promote continuity of care and optimal resource utilization.
The Director will oversee Case Management, Social Work, and Chaplain Services across a 790-bed hospital and will lead a large interdisciplinary team while partnering closely with executive leadership and physicians.
Direct and evaluate departmental operations, including case management models, staffing (skill mix and FTEs), onboarding, competencies, and use of technology.
Provide oversight of Case Management core functions, discharge planning, and care coordination processes.
Ensure compliance with regulatory standards, organizational policies, and SOPs.
Allocate resources effectively to achieve productivity, financial, and quality goals.
Analyze operational data and implement sustainable performance improvement initiatives.
Collaborate with executive leadership, including CFO and Division CM leadership, to achieve strategic objectives.
Establish strong working relationships with CMO, CNO, ancillary leaders, Ethics & Compliance, Legal, and post-acute partners.
Foster physician engagement and strengthen relationships within the post-acute community.
Lead change management initiatives and drive accountability across teams.
Participate in rotating weekend on-call coverage as needed based on hospital operations.

Bachelor’s Degree (Required)
Master’s Degree in Nursing, Health Administration, or Business Administration (Preferred)
Active licensure as one of the following (Required):
Registered Nurse (RN)
Licensed Clinical Social Worker (LCSW)
Licensed Master Social Worker (LMSW)
Minimum 3+ years of acute care hospital case management experience (Required)
Minimum 2+ years of case management leadership experience (Required)
Extensive case management leadership experience preferred
Demonstrated ability to analyze data and independently develop action plans
Strong strategic relationship-building skills with physicians and post-acute providers
Salary Range: $135,004 – $202,555 (based on years of experience)
Incentive Bonus: 17.5%
