About the Facility

A hospital in Nashville is seeking an experienced Director of Case Management to lead hospital-wide care coordination and discharge planning operations.

This Level I Trauma facility serves a high-acuity patient population and manages approximately 17,700+ annual admissions. The Case Management department supports the full continuum of inpatient services, partnering closely with executive leadership, physicians, and interdisciplinary teams to optimize patient outcomes and resource utilization.

Position Summary

The Director of Case Management has overall responsibility for the strategic, operational, and regulatory oversight of hospital-wide case management services.

This leader ensures effective care coordination, discharge planning, length-of-stay management, and compliance with regulatory standards. The Director drives performance improvement initiatives, supports interdisciplinary collaboration, and ensures appropriate clinical resource utilization across the organization.

The role requires executive presence, strong analytical skills, and a proven track record of reducing length of stay, improving throughput, and leading high-performing teams in complex acute care environments.

Responsibilities

Operational Leadership

  • Direct and evaluate all departmental operations, including staffing models, skill mix, onboarding, competencies, and use of technology.

  • Allocate resources effectively to meet productivity and quality goals.

  • Oversee all core Case Management functions and practices.

  • Ensure timely, compliant care coordination and discharge planning processes.

  • Maintain adherence to organizational policies and regulatory standards.

Performance Improvement & Strategy

  • Analyze operational data and identify opportunities to improve length of stay, throughput, and resource utilization.

  • Lead sustainable performance improvement initiatives.

  • Partner with Finance leadership to achieve established financial and operational goals.

  • Manage capacity and support hospital flow initiatives.

Regulatory & Compliance Oversight

  • Ensure compliance with CMS guidelines, accreditation standards, and internal SOPs.

  • Oversee documentation standards and regulatory readiness.

  • Maintain effective collaboration with Ethics & Compliance and Legal departments.

Stakeholder Collaboration

  • Build strong partnerships with CMO, CNO, CFO, ancillary leaders, and physician teams.

  • Lead interdisciplinary engagement to optimize patient transitions of care.

  • Drive change management initiatives across departments.

Leadership & Culture

  • Foster trust, team engagement, and accountability.

  • Provide coaching and professional development to staff.

  • Lead through influence, executive presence, and motivational leadership.

  • Navigate conflict and complex stakeholder relationships with diplomacy.

Qualifications

Education

  • Bachelor’s Degree required

  • Master’s Degree in Nursing, Health Administration, or Business Administration preferred

Licensure

  • RN, LCSW, or LMSW required

Experience

  • Minimum 3+ years of recent acute care hospital Case Management experience required

  • Minimum 2+ years of Case Management leadership experience required

  • Demonstrated success in:

    • Reducing length of stay

    • Capacity management

    • Data analysis and performance improvement

    • Leading interdisciplinary teams in complex hospital settings

Compensation & Benefits

Salary Range: $109,762 – $135,000 (based on experience)
Incentive Bonus: 17.5%
Sign-On Bonus: $10,000
Relocation Assistance: Case-by-case basis
