Location: Bronx, NY 10461
Contract: 13 Weeks
Hours: 5 Days/Week | 10 AM – 6 PM
Department: Emergency Services

Weekly Gross: $2,779.87

Overview
The RN Discharge Planning Coordinator supports safe transitions of care from the Emergency Department through care coordination and utilization review.

Responsibilities

• Lead discharge planning efforts for ED patients
• Perform utilization management reviews
• Coordinate post-acute care resources
• Partner with providers to reduce length of stay
• Maintain accurate care management documentation

Requirements

• Active NY RN license
• 5+ years RN experience
• Experience with discharge planning and care coordination
• ASN/ADN required
• BLS certification

Preferred

• BSN/MSN
• Case Management certification
