RN Transition of Care Specialist – Emergency Services

Location: Bronx, NY 10461
Assignment: 13 Weeks
Schedule: Day Shift | 40 Hours/Week
Department: Emergency Department

Weekly Gross: $2,779.87

Position Summary
This role supports patient transitions and throughput by providing care coordination, utilization management, and discharge planning within the Emergency Department.

Key Duties

• Facilitate patient transitions from the ED
• Perform utilization reviews and care coordination
• Identify discharge barriers and arrange services
• Collaborate with interdisciplinary teams
• Ensure documentation accuracy and compliance

Qualifications

• NY RN License
• 5 years RN experience
• Care coordination and discharge planning background
• ASN/ADN required
• BLS required

Preferred

• BSN/MSN
• Case Management experience
