RN Case Manager – Emergency Services

Location: Bronx, NY 10461
Contract Length: 13 Weeks
Schedule: 5 Days/Week | 8 Hours/Day | 40 Hours/Week
Shift: 10:00 AM – 6:00 PM
Department: Emergency Department – Care Management

Weekly Gross: $2,779.87

Role Summary
The facility is seeking an experienced RN Case Manager to provide care coordination and utilization management services within the Emergency Department. This role supports patient flow, discharge planning, and regulatory compliance.

Key Duties

• Coordinate care and perform utilization review for ED patients
• Collaborate with physicians and interdisciplinary teams
• Facilitate discharge planning and post-acute referrals
• Identify discharge delays and recommend interventions
• Maintain documentation and compliance standards

Requirements

• Active NY RN License
• Minimum 5 years RN experience
• Background in care coordination, UR, and discharge planning
• ASN/ADN required
• BLS certification

Preferred

• BSN or MSN
• Case Management certification
• Prior leadership experience
