Location: Panama City, FL
Position Type: Full-Time | Permanent
Shift: Evening | 12:00 PM – 8:30 PM EST
Weekend Requirement: 1 weekend per 4-week schedule
Hourly Rate: $31.94 – $43.92 (based on experience)
Evening Differential: +$2.50/hr
Weekend Differential: +$2.00/hr
(Differentials are stackable, up to +$4.50/hr)
Sign-On Bonus: $10,000
Paid in 5 installments over 2 years
A leading acute-care hospital in Panama City, FL is seeking a Registered Nurse Case Manager to coordinate patient care, evaluate clinical and psychosocial needs, and ensure the efficient use of hospital resources.
The RN Case Manager supports interdisciplinary collaboration, promotes patient throughput, and facilitates safe transitions across levels of care while maintaining compliance with regulatory and quality standards.
Assess patients’ medical, psychosocial, and discharge planning needs
Develop and coordinate individualized case management plans
Lead and participate in interdisciplinary care team discussions
Evaluate admissions, observation status, and medical necessity
Perform utilization reviews and communicate with third-party payers
Coordinate safe, timely, and effective discharge planning
Identify barriers to care and implement process improvements
Educate patients and families and connect them with community resources
Maintain accurate and timely documentation of care coordination activities
Ensure compliance with regulatory, accreditation, and hospital quality standards
Education:
Associate Degree or Diploma in Nursing (required)
Bachelor’s degree in Nursing (preferred)
Licensure:
Active Florida RN license
Compact license accepted with Florida RN obtained within 90 days
Experience:
Minimum 3 years of acute care case management experience
OR
Minimum 3 years of RN experience in one or more of the following areas:
Med/Surg
Telemetry
Neuro
ICU
PCU
Emergency Department
Certifications (Preferred):
Case Management
Nursing specialty
Utilization Review
