Job Summary

A large academic medical center and regional Level I Trauma Center with a freestanding children’s hospital in Savannah, GA is seeking an experienced RN or LCSW Manager – Case Management.

Reporting to the Director of Case Management, this role provides operational oversight and leadership for hospital-wide case management services. The Manager promotes patient-centered care through effective care coordination, appropriate resource utilization, quality outcomes, and patient throughput while ensuring compliance with organizational policies and regulatory requirements.


Key Responsibilities

  • Provide day-to-day leadership, supervision, and administrative oversight of the case management team

  • Coordinate and manage hospital-based case management services across assigned areas

  • Ensure care coordination practices align with hospital policies, regulatory standards, and best practices

  • Monitor quality metrics, documentation, assessments, and staff interactions to maintain high standards of care

  • Serve as a clinical expert and subject matter resource for case management processes

  • Promote quality outcomes, efficient patient flow, and appropriate utilization of hospital resources

  • Support interdisciplinary collaboration to enhance care transitions and discharge planning


Education & Experience Requirements

  • Bachelor’s Degree in Nursing or Master’s Degree in Social Work (MSW) – Required

  • 3+ years of acute care clinical nursing experience or hospital social work experience – Required

  • 2+ years of supervisory or leadership experience in an acute care setting – Required

  • InterQual experience – Preferred


Licensure & Certifications

  • Active Registered Nurse (RN) license in applicable state(s)
    OR

  • Active Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) license in applicable state(s)

  • Case Management Certification (CCM) and/or InterQual (IQCI) Certification – Preferred
